For social prescribers / professionals
This page is designed as local signposting information, not as an NHS referral system. NHS England says social prescribing link workers connect people to community-based support and work collaboratively with partners to identify gaps in local provision. Digital tools can support this work, but NHS England says they should complement personalised human support rather than replace it.
Hounslow Reach – floating support
Hounslow Council says Hounslow Reach supports residents aged 18+ who may have mental or physical health needs or a learning disability. Support can include maintaining accommodation, budgeting, wellbeing and building community links.
Professional referrals: the Council provides a professional referral route through its Targeted Support Team.
Hounslow Reach detailsUse official directories too
Hounslow Connect is the Council's directory for local services and support. Professionals should cross-check current availability, eligibility and referral requirements before signposting someone.
Hounslow ConnectWhat FelthamDeals can provide
Our aim is to make local information easier to find: service purpose, who it supports, location, cost, accessibility, contact method, referral/self-referral route and last verified date.
Verified local organisations
We are building this section around a simple rule: do not publish an organisation as verified unless its key information has been checked. A listing should identify what the organisation does, who it supports, where and when it operates, how to contact it, whether self-referral is possible, accessibility information where available and the date its information was last checked.
Currently verified official resources
For this launch, the strongest verified resources are official NHS England and Hounslow Council pages and the Council's Feltham Community Hub information.
CHECKED 29 AUG 2026Verification does not mean endorsement
“Verified” means the published factual information was checked against a source. It does not mean NHS, council or clinical endorsement unless an official source explicitly says so.
Out-of-date information
Community services change. If a listing is wrong or has closed, users will be able to report the problem so we can re-check it.
How organisations can be listed
Community groups, charities, voluntary organisations and relevant local services can suggest a listing. FelthamDeals will review the information before publishing it as a verified resource.
